ADHD Assessments

Do you or your child struggle with poor concentration or motivation, have difficulty with organization and deadlines? Undiagnosed ADHD can manifest in your personal and professional life. This evaluation includes a clinical interview, testing, ADHD self report forms, physician consult, family/friend rating scales, written report, and treatment suggestions.

This evaluation process is for adults or youth who have often thought or wondered if they have ADHD but have never been formally assessed or diagnosed.

This evaluation includes a brief summary of evaluation results (2-4 pages) which can be used to share with a provider for medication, schools, etc.

The evaluation includes up to 3 in person observations/meetings providing thorough assessment time.

This evaluation includes a brief follow up session or phone call to discuss results of the written evaluation.
